I am not sure where is SVM coming from. Please let me know how to fix this error.
I would like reinitialize the disk.

You might need to replace it.
What's <vxdisk -o alldgs list> output ?
To initilalize disk with vxvm:
.Label your disk if not done (format etc.)
. vxdctl enable
. vxdisk list (check status of disks)
./etc/vx/bin/vxdisksetup -i disk_10
Did you check the /var/V* logs?
