Solaris boot error

I am getting the following error when booting from ILOM.

ERROR: /packages/afs-file-system: Last Trap: Trap

Below are the steps that I ran:
1-Connect Laptop/PC to the Serial Port
2- Connect power source and wait until you receive the login screen on putty
3- Login with username: root and password: changeme
4- type start /SYS to start the server
5- type start /SP/console to connect to the console
6- Wait until the POST process has finished
7- At the OK prompt type the boot command: boot

Getting the following error:
ERROR: /packages/afs-file-system: Last Trap: Trap

Do you get this error during the loading of the kernel?
Does it fall back to the OK prompt then?

This is obviously a SPARC system (not x86).

I have a number of questions.

Are you the sysadmin? When was the system last booting properly?

Which version of Solaris is it? Do you have the installation media/DVD?

Do you know how many disks are in the system? Perhaps you are trying to boot from the wrong one?

Just typing "boot" at the OK> prompt will attempt to boot from the address set in the ILOM. Perhaps it's not set right?

If you have the installation DVD put that into the DVD drive and at the OK> prompt enter:

OK> boot cdrom -s

This will check out the hardware functionality to check for a fault. It should just drop you to a # prompt. If it does that enter 'format' command and post back the output. It will display how many drives are in the system. Caution: Just quit out of the format command so not to attempt any writes to disk(s) at this stage.
