at least one lp of a logical volume mirror is out of sync

check your vgs with

lsvg volumegroupname

and watch out for this line:

STALE PVs:          0                        STALE PPs:      0

if the stale pp count is > 0, try

varyonvg volumegroupname

or, if it's a cluster with concurrent vgs, run

smitty cl_syncvg
